Photo Booth uses the front camera to display the subject in multiple tiles, each with a different effect, on the iPad screen. Effects include Kaleidoscope, Stretch, X-Ray, and more. The tile in the center of the screen displays Normal view. Aim iPad at your subject to frame your shot. Tap the tile of the effect you want to capture, then do any of the following.

OK. Yes, you must be doing something wrong.
Do you have enough light in the room?
Perhaps you are not stepping out of the picture until you see the the 'Background Detected' message?
Have you searched PhotoBooth > Help for hints?
You don't specify what's wrong, but here are a couple of other common problem solutions:

Photo Booth is trying to open, but as it's trying to load your preferences file, it's corrupted and causing PhotoBooth to get stuck.
You need to quit PhotoBooth and open your 'user'/Library/Preferences folder and look for a file called
com.apple.PhotoBooth.plist
and delete it by dropping it into the Trash.
then reboot then restart PhotoBooth and the file will be recreated, then set your iPhoto preferences again.
If your on Lion, the 'users'/Library/Preferences is invisible, you can use the GO menu option in Finder or use the free TinkerTool to turn on/off invisible items (make sure to turn it off when done)
